Due to popular request, we will once again offer Direct Billing to patients who have coverage through SunLife Financial.

Please let staff know if you are covered through SunLife, along with your policy number and member ID. You can also read through the [Consent and Authorization for Electronic Claims Submission and Assignment of Benefits Form].
